Form MDJ-008 Maryland — Restricted Information Notice for Garnishment Filings
One of the most common reasons Maryland garnishment filings get rejected has nothing to do with the garnishment itself. It's a missing Form MDJ-008.
Under Maryland Rule 20-201.1, any court filing that contains restricted personal identifiers — bank account numbers, Social Security numbers, taxpayer IDs, financial account numbers — must be accompanied by a Notice Regarding Restricted Information. That notice is Form MDJ-008.
When You Need It
You'll most likely need MDJ-008 when filing:
- Form DC-CV-065 (wage garnishment) — if you include the debtor's Social Security number or another restricted identifier
- Form DC-CV-060 (bank account garnishment) — if the filing includes an account number or another restricted financial identifier
- Other post-judgment filings — if the document includes restricted account numbers or identifiers
If your garnishment paperwork doesn't contain any restricted identifiers, you don't need MDJ-008. A bank garnishment filing needs it when it contains a restricted account number or other identifier.
How to File It
MDJ-008 is a one-page form. You fill in the case number, list the types of restricted information included in your filing, and submit it alongside the garnishment request. The form doesn't contain the restricted data itself — it simply alerts the court that the accompanying documents do.
For a paper filing, file MDJ-008 with the garnishment request. If restricted information is necessary in an otherwise public document, also file an unredacted copy marked “unredacted--to be shielded” and a redacted copy labeled “redacted.”
For electronic filing in an MDEC county, submit a separate Notice Regarding Restricted Information filing and attach MDJ-008; check the Restricted Information box on the form when required. MDEC is mandatory for attorneys and available to self-represented filers.
What Happens If You Skip It
The clerk must reject a submission that contains restricted information but lacks the required notice. When restricted information is necessary in an otherwise public document, the rules also require redacted and unredacted copies so the restricted material is not exposed in the public version.
The fix is simple: always include MDJ-008 when your garnishment paperwork contains any bank account numbers, Social Security numbers or other financial identifiers. It takes two minutes to fill out and prevents delays that can cost you weeks.
