Vermont Small Claims Post-Judgment Forms: Complete List and When to Use Each

The Post-Judgment Form Maze

The Vermont Judiciary publishes all court forms for free — but they're scattered across different pages with no guide explaining which forms go together for each enforcement action. Here's the complete inventory of post-judgment forms, organized by what you're trying to do.

Asset Discovery Forms

Form 100-00279 — Motion for a Financial Disclosure Hearing (Small Claims). This is your starting point when you don't know what the debtor owns or earns. Rule 7 requires personal service of the judgment or an acceptance of service; a lump-sum judgment must be unpaid 30 days after service (with a 12-month wait after an inability-to-pay finding unless there is good cause), or an installment must be 30 days overdue. The filing fee is $65. More on financial disclosure hearings.

Form 100-00127 — Financial Disclosure Affidavit. The debtor completes this under oath before the hearing, listing income, expenses, bank accounts, and property.

Wage Garnishment Forms

Form 100-00506 — Motion for Trustee Process (Wage Garnishment). Filed with a $65 fee when you know the debtor's employer. Full process at wage garnishment after judgment.

Form 100-00508 — Employer Disclosure. The employer completes this, reporting the debtor's gross pay, pay periods, and statutory deductions. Due to the court at least 3 days before the hearing.

Bank Garnishment Forms

Form 100-00505 — Motion for Trustee Process (Non-Wage Garnishment). Filed with a $65 fee when you know the debtor's bank. Details at bank account garnishment.

Form 100-00297 — Trustee Disclosure (Non-Wage Garnishment). The bank completes this, disclosing account balances and ownership.

Property Seizure

Form 100-00502 — Writ of Execution. Filed with the clerk when you want the sheriff to seize and sell the debtor's personal property. Returnable to the issuing court within 60 days. See writs of execution.

Forms That Go With Everything

Form 100-00511 — List of Exemptions. Include it with financial-disclosure and trustee-process packets, and with the writ packet. It notifies the debtor of their statutory protections under 12 V.S.A. § 2740.

Form 100-00260 — Certificate of Service (Small Claims). Filed with the financial-disclosure motion and the full-payment notice to document service on the debtor.

Form 100-00128 — Disclosure of Exempt Income. The debtor uses this form to identify exempt income; property exemptions are asserted through the process applicable to the levy or execution.

Judicial Summons — Small Claims — Issued by the court when it schedules a Rule 8 contempt hearing. Send it for personal service with the Financial Disclosure Affidavit (Form 100-00127) and List of Exemptions (Form 100-00511), then file the Return of Service before or at the start of the hearing.

Case Closure

Form 100-00290 — Notification Judgment Has Been Paid in Full. You must file this within 21 days of receiving final payment. No fee. More at satisfaction of judgment.

General Motions

Form 100-00053 — Civil Division Motion. A catch-all for miscellaneous post-judgment requests that don't have a dedicated form.

Form 600-00228 — Application to Waive Filing Fees and Service Costs. Available if you can't afford the $65 motion fee.
