Wisconsin Small Claims Court Forms for Judgment Collection
Where to Find Wisconsin Post-Judgment Forms
All official forms are free on the Wisconsin Court System website at wicourts.gov. Navigate to the Forms section, then look under Small Claims and Circuit Court – Civil categories. Each procedure area has its own instruction packet (form numbers ending in "V") alongside the operative forms you actually file with the court.
The instruction packets are voluntary guides — they explain procedure but are not filed with the court. The operative forms are what you submit to the clerk, serve on the debtor, or hand to the sheriff.
The Core Post-Judgment Forms
Here is the sequence most judgment creditors follow, in the order you'll typically need them:
SC-6090V — Collecting a Money Judgment. This is the master instruction guide covering the entire post-judgment workflow. Read it first for the big picture, then use the specific forms below for each step.
SC-506A and SC-506B — Financial Disclosure Order and Statement. The court issues SC-506A automatically when it enters your judgment. SC-506B is the blank form the debtor must complete within 15 days, disclosing income, bank accounts, real estate, and vehicles. The debtor delivers SC-506B directly to you — it does not get filed in the court record.
SC-508 — Petition and Order for Hearing on Contempt. If the debtor ignores the 15-day financial disclosure deadline, you file SC-508 to start contempt proceedings. The court schedules a hearing where the debtor must explain noncompliance.
SC-6060V — Basic Steps for Docketing a Judgment. The instruction guide for judgment docketing, which creates a lien on the debtor's real estate in that county. You apply to the Clerk of Circuit Court and pay a $5 docketing fee.
Garnishment and Execution Forms
SC-6070V — Earnings Garnishment Instructions. The instruction packet for wage garnishment under Chapter 812, Subchapter II. File CV-421 (Earnings Garnishment Notice) with the clerk and pay the $121 filing fee; the clerk issues CV-422 (Earnings Garnishment) for service.
SC-6071V — Non-Earnings Garnishment Instructions. Covers bank account levies under Chapter 812, Subchapter I. The operative forms are SC-301/CV-301 (Summons and Complaint in Garnishment), CV-302 (Garnishee Answer), and CV-303 (Debtor Answer).
SC-6080V — Execution Against Property Instructions. Covers directing the county sheriff to seize and sell non-exempt personal property under Chapter 815. The operative form is the Writ of Execution issued by the clerk.

Closing and Transfer Forms
GF-129 — Satisfaction of Judgment. Once the debtor pays in full, execute and deliver a satisfaction of judgment. Under Wis. Stat. § 806.21, you have 5 business days after written demand to do so, or face a $100 penalty plus actual damages. Submit the signed form to the clerk with the $5 filing fee.
Transcript of Judgment — To enforce your judgment in a different Wisconsin county, request a certified transcript from the original court's clerk ($14 fee) and file it in the target county ($5 docketing fee).
Tips for Using the Forms
Download every form you might need at the start of your collection process. Having the full set prevents delays when you need to pivot between enforcement tools based on what the debtor's financial disclosure reveals.
When filing any form that contains financial information, redact protected data (Social Security numbers, full account numbers) per Wis. Stat. § 801.19. If the court needs the unredacted version, submit it separately on Form GF-241, which stays sealed from public view.
Our Wisconsin judgment collection guide organizes every form into a step-by-step workflow so you know exactly which form to file, when to file it, and what happens next.
