Arizona Small Claims Court Forms — Which Ones You Need and Where to Get Them
There Is No Single Statewide Form
Arizona doesn't have one standard small claims complaint form that works everywhere. Each county's Justice Court has its own packet, and filing the wrong county's form gets your complaint rejected at the clerk's window.
The state informational portals — AZCourtHelp.org and AZCourts.gov — publish general procedural guides and sample templates, but these are reference materials, not filing-ready documents. You need the packet from the specific Justice Court precinct where you're filing.
County-Specific Complaint Packets
Maricopa County uses the Small Claims Complaint & Summons form (SC 8150-311), downloadable from the Maricopa County Justice Courts website. The packet includes the complaint, summons, and the Notice to Plaintiff and Defendant that must accompany service.
Pima County uses Form CVSCPK1 — the Small Claims Complaint & Summons Packet available from the Pima County Consolidated Justice Court forms page. The packet costs $60; the complaint filing fee alone is $55. It covers the complaint, summons, and all required service documents.
Coconino County and other rural counties use their own standardized forms (e.g., LJSC00002F). Check your county's Justice Court website or call the clerk to confirm which form they accept.
What Every Complaint Form Requires
Regardless of county, your complaint needs:
- Your full legal name and address (the plaintiff)
- The defendant's exact legal name — not a trade name or DBA alone; if suing a business, use the name on file with the Arizona Corporation Commission
- The defendant's address — street address in the Justice Court precinct where you're filing
- The dollar amount claimed (up to $5,000)
- A brief statement of facts — what happened, when, and why you're owed money
- Your signature — unsigned complaints get rejected
The complaint must include copies for each defendant. If you're suing two people, you need the original plus two copies.

Electronic Filing Options
Several Arizona counties now accept small claims filings through AZTurboCourt, the state's eFiling portal. Participating counties include Pima, Pinal, Gila, Mohave, Yuma, La Paz, Apache, Graham, and Greenlee. The system walks you through a guided questionnaire and generates court-approved documents.
AZTurboCourt adds convenience fees on top of the standard filing fee. Maricopa County Justice Courts also have their own online filing pathway, though in-person and mail filing remain available everywhere.
After You File
Once the clerk accepts your complaint, stamps it, and assigns a case number, the clock starts. You have 45 calendar days to serve the defendant and file proof of service — miss that window and the court dismisses your case automatically under ARSCP Rule 5.
The Arizona Small Claims Filing Guide covers the full sequence from choosing the right form to filing proof of service, with a deadline calendar so you don't lose track of the 45-day window.
